In case of being a contact person or having a close contact with a subject person of testing

Main changes
- Health observations are to be recorded in “Health Observation Report Sheet.”
- You are required to fill out “Close Contact Report Form,” if you become a close contact person.
1. If you are not a close contact person, but have (received) a PCR test (including an antigen test) as a “contact person”
Stay-at-home quarantine period
Please stay at home until the test results are confirmed and be sure to report the results to your faculty/department and the Health Administration Center.
Even if the test result is negative, when you are required to take a retest, you shall stay at home until the results of the retest are confirmed.
If the test result is “negative,” please follow the instructions of the medical institution where you took the test. If there are no specific behavioral restrictions, you may return to class/work with caution, taking the utmost infection prevention measures (wearing a mask, washing and sanitizing hands and keeping social distance, etc.).
However, if you become aware of any illness within 7 days of your last contact, please stay at home and contact your faculty/department immediately.

Please download the “Health Observation Report Form” and record your physical condition and temperature.
(The last day of contact is set as the day 0 and continue until the day when the stay-at-home quarantine is lifted)
2. If you have been in close contact with a person who is suspected of having been infected and is subject to a COVID-19 test.
Stay-at-home quarantine period
Please stay at home until the test result of the person to be tested is confirmed.
If the test result of the person subject to testing is “negative,” you may return to class/work with caution, taking the utmost infection prevention measures (wearing a mask, washing and sanitizing hands, keeping social distance, etc.).
However, if you are aware that you are feeling unwell, please stay at home.
* If the test results of a person subject to testing turn out to be “positive,” you become a close contact person, so you shall immediately report to Yamaguchi University using the “Close Contact Report Form” and shall stay at home for five days, starting from day 0 of the close contact day.

Health Observation

(The date of last contact should be the 0th day, at the earliest, until the test results of the person to be tested are revealed (may be extended depending on the situation)).

(1) Contact at a distance of about 1 m without a non-woven mask each other for 15 minutes or more
(2) Long term contact (30 minutes or more) with a COVID-19-infected person while living together or in a car, etc.
(3) Examining, nursing, or caring for a COVID-19-infected person without appropriate infection protection (non-woven mask + goggles or face shield)
(4) Direct contact with contaminants such as airway secretions or body fluids of a COVID-19-infected person.
