{"id":846,"date":"2022-02-10T17:55:26","date_gmt":"2022-02-10T08:55:26","guid":{"rendered":"https:\/\/ds27i1.cc.yamaguchi-u.ac.jp\/~www-yu\/english\/?page_id=846"},"modified":"2022-05-11T16:21:10","modified_gmt":"2022-05-11T07:21:10","slug":"lost_damaged","status":"publish","type":"page","link":"https:\/\/ds27i1.cc.yamaguchi-u.ac.jp\/~www-yu\/english\/life\/immigration_procedures\/lost_damaged\/","title":{"rendered":"Lost, Damaged, or Stained Card"},"content":{"rendered":"<h2>Lost, Damaged, or Stained Card<\/h2>\n<h3>If You Have Lost Your Residence Card:<\/h3>\n<p><a href=\"https:\/\/www.isa.go.jp\/en\/index.html\"><span class=\"btn_bg_green sp_w_100\">Application for Reissuance of Lost Residence Card<\/span><\/a><\/p>\n<p class=\"ti_01\">If your card is lost or stolen, you must report it to the local police station and apply for your card to be reissued at the Immigration Bureau within 14 days of the incident.<\/p>\n<h3>You will need the following:<\/h3>\n<ul>\n<li>Application form for re-issue<\/li>\n<li>Photo (4cm\u00d73 cm)<br \/>\n*Write your name on the back of the photo and glue it to the application.<br \/>\n*The photo must have been taken within the past 3 months. You may not wear a hat, and the photo should be clear with a plain background.<\/li>\n<li>Passport<\/li>\n<li>Permit to engage in activities other than those permitted by the status of residence previously granted (if applicable)<br.\n\n\n<li>Evidence showing you have lost your card (Report of lost property, report of stolen property, report of disaster, etc.)<br \/>\n*If you are unable to provide any of the above evidence, you must turn in a written statement explaining the situation and the reason it was lost.<br \/>\n*There is no fee for replacing a damaged residence card.<\/li>\n<\/ul>\n<h3>If You Have Damaged or Stained Your Residence Card:<\/h3>\n<p><a href=\"https:\/\/www.isa.go.jp\/en\/index.html\"><span class=\"btn_bg_green sp_w_100\">Application for Reissuance of Damaged Residence Card<\/span><\/a><\/p>\n<p class=\"ti_01\">If your residence card is badly damaged, stained, or the information on the IC chip is damaged, you will have to apply for the card to be reissued.
